Short answer
There is no single universal document list -- what you need depends heavily on your basis for residence (work, marriage to a Polish citizen, joining a foreign-resident family member, study, business, etc.). Every route shares a few common elements: a valid passport, a completed application (usually via MOS), a biometric photo, and evidence supporting your specific basis.
What this depends on
The biggest factor is *why* you're applying. A work-based application needs employer documents and Annex No. 1. A marriage-based application needs a marriage certificate (and possibly its translation and authentication). A family-reunification application built around a foreign sponsor needs evidence of the sponsor's own qualifying status, income, insurance, and accommodation.
