## Short answer
15 WORKING days from the date your employment with the entity named in your permit ended.

## Important exception
Your employer has a separate duty to notify the voivode too -- but theirs is counted in 15 CALENDAR days, not working days. These are two different counts. Don't assume your employer's notification covers your own duty, or that the two deadlines land on the same date.
