For the marriage-based route specifically, the general pattern reported across multiple voivodeship offices is: the marriage must have lasted at least 3 years before the application, you must have resided in Poland continuously for at least 2 years immediately before the application on a temporary residence permit granted because of that marriage, and the marriage must not have been entered into to circumvent immigration law -- the office is required to assess this.
