Do I need to exchange my residence card?

It depends on whether your passport renewal actually changed data printed on your card -- a same-data renewal doesn't trigger anything, an actual data change starts a 14-day deadline.

What changed?

Statutory grounds requiring a foreigner to exchange their residence card (or Polish travel/identity document): a change of data shown on the document, a change of appearance making identification difficult, loss, or damage.

Separate consideration: did the reason for your permit change too?

Deadline for a foreigner holding any temporary residence permit to notify the voivode that the reason the permit was granted has ceased to exist.

This is a different, additional duty from the card-exchange question above -- a new passport or address alone does not trigger it, but a broader change (e.g. the relationship or status the permit was granted on ending) might. If this applies to you, it’s worth reviewing separately.
